- Applicant must be a resident of Indiana.
- Qualified equipment must be installed in applicant’s single-family, owner-occupied residence.
- Rebates are for replacement equipment only (qualified equipment must be new). Rebates can not be paid for installations in new construction.
- The installing contractor must be a contractor whose business is located in Indiana.
- All applicable fields must be completed on the form to receive a rebate (installation address is required).
- A copy of the customer’s invoice(s) must be stapled to the back of this form. Invoice must be marked “paid” or show a zero balance. Invoice must show the manufacturer and model number of the unit(s) installed. If you are installing your own unit, you must provide a copy of the receipt for the purchase of the equipment and it must show the manufacturer and model number of the unit installed. If the manufacturer and model number are not located on the invoice or receipt, the applicant must provide documentation of the manufacturer and model number of the unit installed.
- For air conditioner and air-source heat pump rebates, a copy of the Air-Conditioning, Heating and Refrigeration Institute (AHRI) certificate must accompany the application. The AHRI certification should be obtained from the installing contractor.
- Equipment must be purchased February 1, 2010 or later. The rebate form and invoice(s) must be postmarked within 30 days of the product purchase or installation (rebate funds are limited and available on a first-come, first-served basis).
- An eligible applicant may receive more than one rebate, but total rebates are limited to $500 per residence (except for rebates for ground-source heat pumps).
- Please allow 4 to 6 weeks to receive your rebate. Incomplete rebate forms cannot be processed.
- The Indiana Office of Energy Development reserves the right to change or terminate this program at any time.
